In this document we recommend: • Optimization of telehealth services, when available and appropriate, continues to be recommended to minimize the need for in-person services. Ensuring that individuals with disabilities have tools for effective communication is a key part of optimizing telehealth services. • All individuals at higher risk for severe COVID-19 illness should continue to shelter in place unless their conditions warrant in-person healthcare. More detailed information about vulnerable and other populations who may need to take precautions is described below. • For care that cannot be provided virtually, these recommendations — which are part of a series of phased recommendations — may guide healthcare systems, providers, and facilities as they consider delivering in-person care to non-COVID-19 patients in regions with lower or declining- without-rebound, levels of COVID-19.

Non-emergent, Non-COVID care (NCC) should be offered to patients, as clinically appropriate, in localities or facilities that have the resources to provide such care, as well as the ability to quickly respond to a surge in COVID-19 cases, if necessary. Decisions should be consistent with Federal, State, and local orders, and CDC guidance and made in collaboration with State and local public health authorities. Careful planning is required to safely deliver in-person care to patients requiring NCC, and all aspects of care must be considered.

May 11, 2020 - The Atlanta Continuum of Care (CoC) developed a triage-screening tool to assist their shelters and housing programs modify shelter management strategies in light of the emerging COVID-19 concerns. The triage-screening tool focuses on reorganizing the intake workflow to effectively identify symptomatic and asymptomatic people entering congregate sites. In Atlanta, Fulton County Public Health has advised congregate programs to practice social distancing to the maximum extent possible for people who exhibit symptoms. This tool was developed collaboratively with support from the local Healthcare for the Homeless and a local public hospital and provides an example of how a CoC can adapt public health guidance in congregate shelter sites.

The list below captures common decision points and OCHIN recommendations: • When should patients be scheduled with a Virtual Visit or a Telephone Visit? Follow your organization’s guidelines. OCHIN recommends delivering care as a Virtual Visit whenever possible to provide the added benefit of a face-to-face connection between the provider and patient. • What is your organization’s process for clinical triage when scheduling appointments? When scheduling appointments, if the patient has clinical or symptom questions that a scheduler cannot assess or answer, OCHIN recommends following your organization’s existing clinical triage process or create a routable Telephone Encounter for a nurse or provider to return a call to the patient. Remember to provide the patient an expected call-back timeframe. • What if the patient is late to the appointment? If the patient is more than five minutes late for the visit, OCHIN recommends attempting to call the patient at the primary phone number listed in the patient’s Electronic Health Record

• What if the provider will be late to the appointment? If a provider will be late for the visit, the patient should be notified by a nurse, MA, or other clinical support staff person. Similar to an in- person visit, consider giving the patient the option to reschedule.

May 15, 2020 - Community- and faith-based organizations (CFBOs), working together with schools, businesses, healthcare systems, and state, local, tribal, and territorial health departments, have an important role in slowing the spread of diseases, especially among high risk populations.

Guidance for CFBOs is organized into three categories based on the level of community transmission: 1) when there is no community transmission (preparedness phase), 2) when there is minimal to moderate community transmission, and 3) when there is substantial community transmission. Guidance is also provided for what to do when a person who is a confirmed COVID-19 case has been in your facility, regardless of the level of community transmission.

All decisions about implementing these strategies (e.g., alteration or reduction of services, event cancellations, other social distancing measures) should be made locally, in collaboration with local health officials who can help determine the level of transmission in the community.
